If your insurance is not listed, a superbill can be provided for you to submit to your insurance for possible reimbursement.
We recommend you call your insurance (the number on the back of your card) and ask them these questions to help you get the most out of your benefits.
Do I need a
Referral?
A referral is needed to bill insurance.
Private pay clients do not require a referral.
What diagnosis is Covered?
Not all insurances cover all nutrition services. We verify insurance benefits as a service to you.
You can also call your insurance to check your benefits. If a diagnosis code is requested, provide ICD Z71.3 (Nutrition Counseling & Monitoring).
CPT codes for medical nutrition therapy are 97802 and 97803.
What if Insurance Denies My Visit?
You’ll be notified and billed for our cash pay rate of $200 for an initial visit and $150 for a follow-up. Payment plans available upon request.
What about My Deductible?
In the event of a deductible, you will be billed the full amount of your deductible rate per your insurance. It can range between $250 to $400 for an initial visit and $200-250 for a follow up visit.
If you are using an HSA or FSA account, an invoice and services billed is available upon request.
